These logs allow Veterans to record daily symptom frequency, severity, and functional impact in a consistent format, offering additional clarity regarding how symptoms affect daily life.
Symptom Logs are designed assupplemental tools to assist in describing symptom patterns and are not intended to replace medical records or provider evaluations. When used alongside medical and service documentation, they may help provide helpful context regarding symptom persistence and functional impairment.
The 14-day Symptom Logs help Veterans:
Accurately track symptoms over a defined period
Describe symptom consistency, severity, and fluctuations
Document functional impact on daily activities
Provide clearer insight into the Veteran’s day-to-day experience
These logs can be especially helpful when symptoms are intermittent, variable, or difficult to describe retrospectively.

Heart Conditions
General / Multi-Symptom Tracking
Headaches and Migraines
Gastroesophageal Reflux Disease (GERD / Heartburn)
Each log is structured to allow concise daily entries while capturing meaningful detail relevant to symptom impact.
Symptom Logs arenot medical diagnoses and do not replace formal medical evaluation or treatment.
Completion of a Symptom Logdoes not guarantee a Nexus Letter or VA claim outcome.
