Symptom Logs

To support clear and accurate documentation of medical conditions,Nexus Veteran Consultants provides optional 14-day Symptom Logs to help Veterans describe their lived experience with ongoing symptoms. These logs allow Veterans to record daily symptom frequency, severity, and functional impact in a consistent format, offering additional clarity regarding how symptoms affect daily life.

Symptom Logs are designed assupplemental tools to assist in describing symptom patterns and are not intended to replace medical records or provider evaluations. When used alongside medical and service documentation, they may help provide helpful context regarding symptom persistence and functional impairment.

Purpose of Symptom Logs

The 14-day Symptom Logs help Veterans:

  • Accurately track symptoms over a defined period

  • Describe symptom consistency, severity, and fluctuations

  • Document functional impact on daily activities

  • Provide clearer insight into the Veteran’s day-to-day experience

These logs can be especially helpful when symptoms are intermittent, variable, or difficult to describe retrospectively.
